Job AdvertThis is a part‑time, fixed‑term position of 22.5 hours per week, ending December 2027. Working days are Tuesday, Wednesday, and Thursday. A valid driving licence and access to a vehicle are essential.
About Healthier MeWe aim to:
- Improve health outcomes for people with a learning disability.
- Empower those we support and our staff with better tools for health and wellbeing.
- Make community health offerings more accessible.
You will work within a team of Healthier Me Coaches across England, Wales, and Northern Ireland, both independently and collaboratively.
Key Responsibilities- Line‑manage at least one Healthier Me Champion.
- Manage a caseload of approximately 100 individuals, supporting them to set and achieve personalised health goals.
- Ensure all contractual outcomes linked to your caseload are achieved, captured and documented.
- Deliver workshops and training to Mencap teams, people with a learning disability, and community members.
- Implement community‑led approaches and co‑production.
- Provide sign‑posting, support and tools to individuals, carers and staff.
- Take responsibility for own professional development and continue to improve performance.
- Develop relationships with stakeholders and gather feedback.
- Complete required internal and external reporting accurately and on time.
- Generate community opportunities with local organisations.
- Escalate any practice issues or safety concerns following organisational policies.
- Be passionate about making health and wellbeing accessible and have excellent communication and relationship‑building skills.
- Show enthusiasm, flexibility and commitment to the team.
- Understand community‑led ways of working.
- Be aware of duty of care, safeguarding and critical incident reporting procedures.
- Attend meetings to support collaboration across Mencap.
