Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in long-term care. This role requires a strong understanding of medical procedures, excellent communication skills, and the ability to work effectively within a multidisciplinary team. The LPN will play a vital role in ensuring patient comfort, safety, and well-being through skilled nursing interventions and personalized care plans.

Duties

Administer medications and treatments accurately following physician orders and established protocols

Monitor resident vital signs and observe for changes in condition, reporting significant findings promptly

Assist with patient assessments, including wound care and catheter management

Provide specialized care for residents with dementia/seniors.

Document patient care activities thoroughly using electronic health records such as Point Click Care

Collaborate with physicians, nurses, therapists, and family members to develop and implement individualized care plans

Educate residents and their families on health management and medication use

Ensure compliance with healthcare regulations, safety standards, and infection control protocols

Experience

Prior experience in long term environments is highly preferred

Experience in dementia care/senior care,

Knowledge of medical terminology and medication administration procedures is required

Familiarity with electronic health record systems - PCC is desirable

Proven ability to handle resident care in diverse clinical situations including pain management and case management

Strong communication skills and the ability to work effectively within a multidisciplinary healthcare team
