The Nurse Practitioner will work as an autonomous, highly skilled clinician within Collingwood Health Group, providing advanced clinical assessment, diagnosis, and management of patients presenting with acute and undifferentiated conditions. The post holder will deliver high-quality, patient-centred care through same-day and urgent appointments, including the management of minor illness, minor injury, and acute exacerbations of chronic disease.

Working within their scope of practice and professional regulatory framework, the Nurse Practitioner will independently prescribe, request and interpret investigations, initiate treatment plans, and ensure appropriate follow-up or escalation of care where required.

The role requires strong clinical decision-making, effective risk management, and the ability to work both autonomously and collaboratively within a multidisciplinary primary care team. The post holder will contribute to service development, clinical governance, and continuous quality improvement, while maintaining accurate clinical records and upholding the highest standards of professional practice, confidentiality, and patient safety.

The Nurse Practitioner will support health promotion, patient education, and preventative care initiatives, contributing to improved patient outcomes and enhanced access to timely primary care services.

Main duties of the job

The postholder delivers high-quality, patient-centred clinical care, independently assessing, diagnosing, and managing patients with acute and undifferentiated conditions. This includes providing same-day and urgent appointments for acute illness and minor injuries, prescribing medications, requesting and interpreting investigations within scope of practice, initiating treatment plans, giving safety-netting advice, and arranging appropriate follow-up. The role involves recognising deterioration, escalating care when required, and referring patients to secondary or emergency services.

Management of acute exacerbations of chronic diseases such as asthma, COPD, and diabetes is required.

In addition, the postholder provides patient education and health promotion, ensuring patients and carers understand diagnoses, treatment options, and self-care strategies, while supporting preventive care and early intervention. The role requires effective multidisciplinary working alongside GPs, nurses, allied health professionals, and administrative staff, contributing to clinical handovers, case discussions, and mentoring colleagues where appropriate.

The postholder maintains accurate, timely clinical documentation, adheres to national guidelines and professional standards, and participates in audit, quality improvement, and risk management activities. Ongoing professional development, revalidation, and maintenance of registration are essential components of the role.
