This nursing position serves as a home visitor to Healthy Start clients. The Nurse Home Visitor is responsible for providing face-to-face visiting services to Healthy Start clients either in the home, office, or clinical setting. The Healthy Start Nurse will deliver education, care coordination, screening and interventions/referrals, follow-up in accessing prenatal care and other needed services, anticipatory guidance and ongoing support to participants who have high risk factors associated with an increase in preterm delivery, low birth weight delivery and/or infant mortality.
Services are provided to mitigate or eliminate these risk factors and address social determinants of health that contribute to adverse birth outcomes and developmental delays.
Responsibilities of the Healthy Start Nurse home visitor include using nursing assessment skills and judgment to promote positive health outcomes. This may include educating the client to gain a better understanding of medical conditions, medications, recommended schedule for immunizations and well-child checkups. The nurse has a responsibility to teach the client how to communicate with physicians and advocate for themselves and their children.
The nurse has a responsibility to act as a liaison with medical staff, counselors, and pharmacists to promote positive health outcomes. The nurse has a responsibility to provide trauma informed care to pregnant women, children birth to aged 3, and their families. The nurse’s focus is on linking pregnant women with prenatal, post-partum, inter-conception care, and family planning services. The nurse should provide wellness education and assist families to identify and achieve long-term health goals.
The nurse is responsible for promoting strong parent-child attachment and coaching parents on learning activities that improve their target child’s development.
Nursing responsibilities include case management of a large Healthy Start maternal and infant caseload and the assurance of continuity of care for clients. Many are high-risk Healthy Start families. The cases may include family violence, substance abuse, marital conflict, mental health, family dynamic issues and lack of health promotion orientation. The nurse will utilize good nursing assessment skills and judgment in referring clients for outside services.
The nurse will collaborate with other agencies while maintaining client confidentiality. This position reports to the Program Administrator-SES.
- Completes psychosocial assessments, planning and case management within the scope of (social work/nursing) practice utilizing state protocols, Healthy Start Program Standards and Guidelines and supervisor’s instructions.
- Demonstrates an in-depth knowledge of the health needs, both physical and emotional of pregnant women and babies as evidenced by the ability to deliver health promotion education, disease prevention education, and health protection services. In addition, performs psychosocial assessments with participants in the Healthy Start Program that are at risk for poor outcomes.
- Provides an assessment of environmental; behavioral and social factors that impact on the pregnant woman and/or family’s health and ability to function.
- Provides education to Healthy Start clients and family members on various health issues related to pregnancy and parenting.
- Provides inter-conception education using a standardized curriculum.
- Provides clarification of and assistance with access to the health care delivery system.
- Refers clients to services based on needs identified during assessment and mutually established goals.
- Maintains necessary computer and time management to provide accurate, timely electronic record documentation of all Healthy Start activities in accordance with timelines necessary to meet Healthy Start contract requirements.
