Overview
Primary Job Function:
The Qualified Intellectual Disability Professional Residential Support Manager provides case management, support, training and advocacy to individuals residing in the Life Skills Unlimited home living program.
- Coordinate supports to meet the needs of individuals with intellectual disabilities and related disorders.
- Follow all company policies and procedures to attain the mission of Life Skills Unlimited.
- Coordinate individual services and support with the person and Residential Training Specialists, including transportation, vocational programs, counseling, community access, etc.
- Develop, author, and implement habilitation and service plans for individuals on caseload.
- Schedule and attend all individual PCPs.
- Schedule and complete home visits to each location monthly to review and evaluate home environment and required documentation.
- Conduct QA evaluations on each home quarterly to prepare for annual QA with DRC.
- Oversee staffing schedules ensuring staffing meets the contractual agreements between Life Skills Unlimited, the individuals and/or their guardians, and ADSD.
- Supervise staff and schedule training for residential training specialists for week one.
- Documentation:
Complete assignments as directed by any management team member; work with Records Maintenance Coordinator to maintain individual clinical records, program books and back files for each person served; review and calculate monthly habilitation data sheets, medication records, daily logs, social calendars, and pertinent information to complete a comprehensive monthly report on each individual. - Collaborate with the CEO to complete paperwork to support the needs of the individuals (Med Clinic, HRC/BIC, Denial of Rights, General Event Reports, etc.).
- Complete case log documentation for each individual every month to reflect services provided during the preceding month.
- Reports to the agency CEO and works as a management team member.
- Maintain family/guardian and other agency contact information.
- Report concerns related to individual health, medical, nutrition, meal planning or grocery shopping to the Medical Coordinator Lead and the CEO.
- Communicate medical and support information to the work/school setting.
- Assist Records Maintenance Coordinator to communicate all individual health information following medical/dental appointments to the Health Care Consultant and DRC Service Coordinator.
- Skills/Expertise:
One (3) years of experience in working in a residential program for people with intellectual disabilities and related disorders. - Bachelor's degree in a "Human Services" field.
- Minimum of one (1) year supervisory experience.
- Knowledge and understanding of using a computer including Word and Excel.
- CPR and First Aid certification.
- Pre-employment Drug Testing.
- Criminal background check with no convictions.
- Valid, unrestricted Nevada driver's license and clean DMV record.
- Proof of current liability insurance and vehicle registration.
- Effective communication with individuals, families, coworkers, contract agencies and community providers.
- Knowledge and skills in financial management; good organizational and time management skills; ability to self-initiate responsibilities; ability to supervise and train support staff.
- Work flexible hours including weekends and evenings.
- Complete required training to maintain employment status.
- Physical Demands:
May involve climbing, balancing, stooping, kneeling, crouching, talking, reaching and manual dexterity. May involve standing/walking/pushing/pulling/carrying. Hearing, speech, and vision requirements; daily computer use. - Strength:
Sedentary to heavy exertion; ability to exert 10-100 pounds occasionally. Mobility to work in a typical residential setting and use standard home appliances. - Examples of Equipment/Supplies:
Thermometers, first aid supplies, blood glucose monitoring supplies, telephones, fax machines, copy machine and general home appliances including stove, oven, microwave, vacuum cleaner, dishwasher, clothes washer/dryer, etc. - Note:
