• Provides resident-centered nursing care and daily living assistance under the supervision of a registered nurse (RN).
• Delivers age-appropriate care related to the resident's physical and psychological needs as per the care plan.
• Adheres to infection prevention and control policies and personal protective equipment use.
• Assists with basic health care needs including bathing, toileting, grooming, dressing/undressing, and vital sign recording.
• Provides psychosocial support and other personal care to assigned residents.
• Assists residents with transfers, repositioning, walking, range‑of‑motion and passive exercises using correct techniques and equipment.
• Completes food preparation tasks and supports a positive dining experience.
• Assists residents with meals and snacks, water, and supplements as appropriate.
• Supports nursing staff with procedures and documents resident interactions and outcomes.
• Communicates changes in residents' conditions and concerns to the charge nurse.
• Collects non‑invasive specimens and vital signs.
• Provides housekeeping and laundry services to maintain residents’ living areas.
• Assists with wellness and recreational programs and encourages participation.
• May participate in departmental/organization committees and task forces as needed.

• High school graduate or GED preferred, but not required.
• Minimum age of 16.
• Prior clinical or nursing assistant experience in long‑term care preferred.
• Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state‑certified CNA within four months of starting the position by enrolling in a training program or the process of obtaining certification through endorsement.
• Consistent yearly competency testing and maintenance of continuing education credits (minimum 12 hours annually).
• Maintains all required department‑specific certifications.
• When applicable, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
• Possible classification as a Sanford Category II Driver requiring a valid driver’s license and meeting medical qualifications per Sanford Driving Policy and leadership request.
