Help the underserved at an IHS in Fort Yates, North Dakota.

  Fort Yates, ND
• Dates coverage is needed: ASAP - 3 months with option to extend (flexible on length)
• Schedule/Shifts Times:
  Monday - Friday, 8:00a - 4:30p or 4 x 10, depending on the staffing need once credentialed.
• Call Requirements:
  None
• % inpatient / % outpatient:
  Outpatient only
• Patient Volume: 6-8 patients per day average
• Patient Ages: all ages, majority adults
• Types of Cases:
  General Dentist, no major procedures required, only procedures Dentist is comfortable with, minor extractions required
• Support Staff: yes - (2 Dental Assist., searching for Dental Hygienist)

• Type of Charting: EMR - Dentrix
• Requirements: BLS (through American Heart), DEA (fee paid, schedules 2-5)
• Reason for Need: staff shortage
• Credentialing Timeframe: 4-6 weeks on average

• Rate Recommendation: $110/hour plus travel and lodging

• NPDB List any and all background
• Current CV in month/year date format
• Complete and sign additional forms
• Life support certification: BLS (through American Heart Assoc)
• Copy of DEA (must be fee paid, schedule 2-5)
• State license (must present all active licenses)
• Three (3) references for docs must be peer level or supervisory from the past 12 months COVID shot required

Closest lodging is in Bismark, ND (approx 1 hour commute).

There is a casino 15-20 min away, but the rooms do not have kitchenttes.

They have a lunch fridge and a local cafe in the facility.

4 provider chairs and 1 hygiene chair.

5-8 provider patients with 3-4 hygiene exam appts.
