Overview

The Captain James

A. Lovell Federal Health Care Center (FHCC) in North Chicago, Illinois is a VA and DoD dually integrated healthcare center providing care to Veterans, Active‑Duty Military Personnel, and their families. We seek an Advanced Practice Nurse (APN) Primary Care Nurse Practitioner (NP) to join our compassionate whole‑health care team.

Major Duties
  • Independently manage a panel of patients with acute and chronic health conditions, emphasizing health promotion and disease prevention using evidence‑based practice.
  • Develop and implement individualized plans to achieve optimal population health and disease management.
  • Demonstrate leadership and collaborate with the Primary Care Team and other interdisciplinary team members.
  • Order and interpret labs, tests, and diagnostic studies.
  • Prescribe pharmacologic and non‑pharmacologic interventions.
  • Initiate referrals and consults to specialty services.
  • Provide education to patients and their families.
  • Assess and adjust the plan of care as needed to achieve optimal patient outcomes.

Qualifications
  • Full and unrestricted license as a Nurse Practitioner from any U.S. state or territory.
  • Advanced Practice Nurse (APN) Certified NP with primary‑care experience.
  • U.S. citizenship (non‑citizens may only be appointed when no qualified citizens are available).
  • Proficiency in basic written and spoken English.
  • Valid driver’s license or REAL‑ identification.
  • Must pass a pre‑employment physical examination and participate in the seasonal influenza vaccination program.
  • Background and security investigation required.
  • May be required to serve a probationary period.
Work Schedule

Monday to Friday, 7:00 am – 3:30 pm. No telework or virtual work. No relocation incentives.
