Overview

OVERALL JOB PURPOSE:
The primary responsibility of the Residential Care Manager is to provide comprehensive case management services to at-risk youth residing in residential group care settings. The Residential Care Manager administers assessments, coordinates treatment teams, develops and monitors progress toward individualized care plans, facilitates therapeutic and enrichment programming, delivers evidence-informed and trauma-responsive interventions, and ensures accurate documentation and data management. The position operates under the supervision of the Director of Residential Care and Compliance.

This role has primary responsibility for service coordination, care planning, documentation oversight, and collaboration with internal and external partners. This position reports to the Director of Residential Care and Compliance.

Scope:
Residential Care Manager services include but are not limited to assessment, service planning, monitoring, coordination, and supervision of family visits and family engagement, group activities, community referrals, and educational and enrichment planning, coordination, and support. Services are based on youth needs and residential staff needs and are delivered in accordance with agency standards, licensing requirements, and program expectations.

This position is also expected to document monthly progress notes in Landmarks for Families  data management system and share all required information regarding youth with the South Carolina Department of Social Services (SCDSS).

  • Build Rapport:
    Establish a professional relationship and be a consistent role model to youth being served. Communicate and uphold Youth Rights and Responsibilities. Promote effective communication between staff and youth.
  • Administer a variety of assessments as required to identify needs and monitor progress of youth throughout placement.
  • Help youth access basic needs such as clothing, dental and hygiene supplies, medical care, and other supports as needed. Assist with transportation services as required.

  • Develop a comprehensive Individual Care Plan for each youth within the first 30 days of admission that outlines various goals. Review and update care plans in accordance with service standards and required timelines.
  • Meet weekly with each assigned youth to evaluate progress toward goals, adjust services as needed, set new goals, track growth, make referrals, and provide ongoing case management services in accordance with service standards.
  • Deliver evidence-informed and trauma-responsive interventions that support emotional regulation, accountability, and skill development while maintaining clear expectations and structure.
  • Develop and coordinate enrichment, educational, and independent living skill-building activities designed to build resiliency, job readiness, and life skills through groups, workshops, and structured sessions.
  • Refer and link youth to community-based programs and services. Serve as a liaison between youth and community partners, service providers, and employers as appropriate.
  • Engage family members and caregivers as appropriate. Participate in all Foster Care Review Board meetings. Supervise and track all family visits and coordinate opportunities for positive family engagement.
  • Serve as a school advocate to monitor grades, attendance, and school behavior. Support youth in setting and achieving academic goals and adjust care plans to ensure educational growth.
  • Assist youth with obtaining and maintaining necessary legal and personal documents when applicable.
  • Participate in monthly treatment team meetings and collaborate with multidisciplinary partners.
  • Complete and submit all required documentation in accordance with service standards, including monthly progress notes, and ensure all assigned youth profiles and records are accurate and up to date in the agency s data management system.
  • Training/Ongoing Professional Development
  • Participates in internal meetings and obtains required training such as agency orientation, CPI, CPR/First Aid, documentation standards, medication administration training, and any other trainings deemed appropriate.
  • Actively seeks out professional development opportunities that strengthen skills in assessment, intervention, and service delivery for the population being served.
  • Provides individualized guidance and support to direct care staff, as needed, to reinforce effective, consistent, and developmentally appropriate behavioral interventions for youth
  • Teamwork, Quality Improvement, and Cultural Sensitivity
  • Promotes the spirit of teamwork and collaboration across programs. Assists with crisis response, problem-solving, Prudent Parenting decision-making, and shift support as needed to ensure youth safety and program continuity. Participates in a required on-call rotation
