Position Summary

The Case Manager
- Student Needs & Community Partnerships is a full-time staff member who reports to the Director of Student Support Services. The Case Manager
- Student Needs & Community Partnerships will:
Communicate, coordinate, refer, and track services for students who demonstrate need. Knowledge of social service delivery systems, campus services, and community services. Create a systemic response to student need, connect students to community-based services and college-specific resources. Develop processes for case management, create and maintain a web-based community resource database. Collaborate with institutional partners with existing services. Other duties as assigned.

Required

Education/Experience
  • Bachelor’s degree in counseling, education, human services, social worker or related field, and at least four (4) years of related human services experience; or, an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
Preferred Education/Experience/Skills
  • Master’s degree in Social Work required. Licensure preferred.
  • Two years of experience in educational counseling/social services environment.
  • Knowledge of social service agencies, eligibility of services and processes to apply.

Skills and Abilities

Excellent communication, interpersonal, and organizational skills are required. Must have good presentation skills. Ability to work under stress, to work independently or in a team environmnent, and deal with the public and other employees in a professional manner. Strong computer skills. Must be able to assure confidentiality, secure documents and sensitive records. Some evening and weekend work may be rquired. Travel between campuses will be required.

Must provide own transportation.

Duties and Responsibilities
  • Coordinates assigned program functions and services, which may include counseling students; determining student eligibility for services; locating relevant academic resources and/or social services for students; ensuring alignment with overall College goals; performing other relevant duties.
  • Manages a caseload of students referred to the Student Wellness Center;
    Communicate, coordinate, refer and track services. Respond systematically to student needs.
  • Develop and maintain community resource database for referrals in the local community and internal support. Insure ongoing accuracy of content and contact information.
  • Knowledge of social service delivery systems, campus services and community services. Facilitate relationships with off-campus agencies and resources to connect student to long-term support.
  • Deliver and facilitate workshops/presentations to students, faculty and staff on topics relevant to student wellness, advocacy, and academic support topics.
  • Collaborate and consult with relevant departments across campus regarding services, access, and care to provide a comprehensive network of support for students in need.
  • Assist with student satisfaction surveys and other methods for data collection and assessment of Basic Needs services. Make recommendations for continuous improvement. As needed, assist in the development and implementation of Basic Needs policies and procedures.
