A well-established independent primary care practice in Shallotte, North Carolina is seeking an experienced Family Nurse Practitioner (FNP) to join our collaborative healthcare team. Our practice follows the Patient-Centered Medical Home (PCMH) model of care — providing coordinated, comprehensive, and preventive healthcare that focuses on long-term patient relationships and whole-person wellness. • Full-time, 100% outpatient family medicine role
• Work alongside a team of experienced team of providers
• Focus on preventive care, chronic disease management, and patient education
• Manage acute and routine primary care visits
• State-of-the‑art facility equipped for in‑office procedures, digital x‑rays, and lab testing
• Support from an experienced clinical and administrative team

• Provide comprehensive primary care services for patients across the lifespan
• Perform physical exams, diagnose and manage acute and chronic conditions
• Order and interpret diagnostic tests, prescribe medications, and develop care plans
• Promote wellness, disease prevention, and patient education
• Collaborate closely with physicians and care teams to ensure continuity of care
• Document all encounters accurately in the Electronic Health Record (EHR) system
• Participate in quality improvement initiatives consistent with PCMH standards

• Current licensure as a Family Nurse Practitioner in the State of North Carolina (or eligibility)
• Graduate of an accredited Nurse Practitioner program
• Minimum of 1–2 years of primary care experience preferred
• Strong clinical judgment, communication, and interpersonal skills
• Commitment to patient-centered, evidence-based care
• Proficiency with EHR systems (Athenahealth, Epic, or similar platforms)
