Hours: Every Saturday & Sunday. Weekend option is a salaried position paid for 40 hours/week with on-call requirement.
Must be available for orientation and training M-F 8-5 for the first 90 days.

SummaryDescription:
The Home Health and Hospice Nurse, RN, Baylor is responsible for working from 5pm Friday thru 8am Monday and taking call as assigned during these hours, administering skilled nursing care to patients requiring intermittent, professional nursing services to include Home Health and Hospice patients.
- Registered Nurse currently licensed to practice nursing in the state of North Carolina. Minimum of one-year experience as a professional nurse in an acute care setting.
- Maintain current CPR certification (BCLS)
- Previous experience working with terminally ill patients preferred
- Basic computer skills/knowledge and experience in electronic medical records
- Ability to understand and communicate well with patients, family, staff, physicians and the public
- Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public
- Must be competent to perform delegated tasks to facilitate positive patient outcomes as indicated in the patients plan of care
- Possess valid Driver's License and reliable transportation
- Conducts the admission of the patient to the Agency by obtaining medical history, including Oasis when needed, particularly as it relates to the present condition, from the patient and/or family member(s); perform a physical assessment of the patient; assess the patients support systems to determine what assistance will be available from family members and their ability to meet the patients immediate needs upon admission and throughout care;
assess the patient, family member(s) and home situation to determine what health teaching will be required; assess the patients condition and home situation to determine if other home health services are needed; interpret nursing and other services of the Agency to patients and families as part of planning for care. - Develop and implement the patient care plan in cooperation with other Agency professionals and paraprofessionals who will be providing care to the patient in Matrix Care.
- Manage assigned cases and provide skilled nursing care as outlined on the physicians plan of treatment and in accordance with the Agencys policies and procedures.
- Observe signs and symptoms and report to the physician and other applicable personnel, reactions to treatments, including drugs and changes in the patients physical or emotional condition and outcomes of intervention. Ensure ongoing evaluation of patient/family response to care.
- Adequately educate, teach, supervise, consult, and counsel the patient / family members regarding the nursing care needs, other related problems of the patient at home, the disease process, self-care techniques, end-of-life care, and processes for dealing with issues of ethical concern. Also provides specialized hospice training to other staff, family members and informal caregivers to ensure adequate care.
