Salary and Benefits

Base pay range: $44,600 – $61,400 per year

Rate per visit/unit: $28.00 – $40.00

Estimated full‑time (40 hours) pay: $53,000 – $73,000 per year based on location and experience

• Provide professional nursing services under the direction of an RN in compliance with the state's Nurse Practice Act, licensure/certification requirements, healthcare standards, and Center Well Home Health policies.
• Deliver skilled nursing interventions for treatment, rehabilitation, and preventive care, using a holistic approach and therapeutic education per the plan of care.
• Apply accepted clinical practices to facilitate problem resolution and achieve individualized patient goals.
• Consult with the RN Supervisor on plan adjustments and communicate physician orders (when permitted by state law) to the RN Supervisor.
• Implement a systematic, goal‑driven approach to the nursing plan.
• Maintain timely contact with patients, physicians, clinical managers, and team members regarding patient/family needs and status changes; participate in care coordination and discharge planning.
• Maintain professional conduct regarding confidential information; share information only when legally and ethically necessary for patient care.
• Ensure availability of supplies, materials, and equipment for safe, effective care delivery.
• Prepare, submit, and maintain required documentation; document visits and shifts on day of service.

• Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• Minimum of two years of LPN/LVN experience in a clinical setting, preferably home health or hospice.

40 hours per week (full‑time)
