Position Summary:

The RN – Special Care Manager is responsible for meeting the needs of memory care residents and managing activities within the special care unit. You’ll coordinate care plans, follow up with medical providers, support residents’ socialization, encourage purposeful living, and ensure compliance with all applicable regulations and company policies.

Essential Duties and Responsibilities:

Clinical & Care Coordination:
  • Develop, implement, modify, and communicate all resident assessments and care plans.
  • Coordinate resident assessments, care plans, and ongoing evaluations in compliance with state licensing requirements (e.g., 10A NCAC 13F/G or equivalent).
  • Supervise and support the care team (Med Aides, CNAs, Personal Care Assistants) to ensure compassionate, professional delivery of care.
  • Conduct initial and periodic resident assessments, medication reviews, service‑plan updates.
  • Maintain accurate, timely, and compliant resident records and documentation.
  • Monitor residents’ health conditions and communicate changes promptly to physicians, families, and leadership.
  • Coordinate care with outside agencies (home health, hospice, therapy providers) as needed.
  • Participate in prospective resident move‑in assessments, resident/family care conferences, and respond to physician/clinician inquiries.
  • Ensure changes in resident conditions are evaluated, reported, monitored, documented and followed up to assure interventions are implemented and outcomes achieved.
Operations & Regulatory Compliance:
  • Partner with the Executive Director to ensure appropriate staffing, training and scheduling within the care department.
  • Create and maintain the monthly care‑staff schedule in the electronic scheduling platform, ensuring adequate coverage based on state rules, regulations and departmental budget—including arranging replacement staff when necessary.
  • May work in the capacity of caregiver and/or Medication Aide to meet staffing ratios and regulatory compliance.
  • Conduct resident rounds to monitor and evaluate the quality and appropriateness of care.
  • Oversee medication‑management systems: ensure medications are current, properly stored, properly disposed when discontinued, and in compliance with physician orders and regulations.
  • Conduct quality assurance audits, infection control checks, incident follow‑up, and support clinical aspects of state/federal survey processes.
  • Audit resident records weekly to ensure documentation reflects compliance with regulatory care requirements, including interventions/outcomes.
  • Audit medication carts weekly to ensure compliance with state and company guidelines.
  • Pull compliance reports (e.g., Matrix Care/Yardi) daily and follow up on non‑compliance areas.
  • Maintain current knowledge of OSHA and state regulations; routinely monitor community compliance with regulations related to resident care.
  • Participate as a member of the community’s leadership team; attend daily stand‑up meetings with community leadership and conduct daily stand‑up meetings with care staff to relay pertinent resident care and wellbeing information.
