RN Case Manager

The home health/hospice RN will case manage and provide skilled nursing services for patient visits. This will include coordination of home care services to patients, supervision of CNAs and PRN nurses in the home, and coordination of care between home care disciplines (PT, OT, ST, CNA, MSW).

Sign-on bonus of $10,000. $5,000 paid at the end of each completed year, and must work a minimum of 32 hours per week.

• Must have graduated from an accredited school of nursing and have a current RN license to practice in NC.
• Experience:
  
  Must have a least one year of experience as an RN in a hospital/home care/hospice/public health setting.

  Valid NC driver s license and reliable transportation.
