A Home Care RN Case Manager coordinates comprehensive patient care in the home, assessing needs, developing individualized plans (POCs), and acting as a liaison between patients, families, doctors, and insurers to ensure quality, cost‑effective treatment, often supervising other home health staff and providing direct skilled nursing care.

• Responsible for coordinating home care services which includes establishing and maintaining the patient’s plan of care.
• Customer service and financial management of your assigned caseload.
• Conducts admissions, recerts, ROC, and discharges for the assigned case load.
• Conducts routine nursing visits as needed to assess patient needs.
• Coordinate patient care between the patient, caregiver, physician, and other appropriate disciplines.
• Responsible for the completion of all home health documentation.

• Graduate of an accredited school of nursing required.
• Minimum Associate's or preferred Bachelor's degree in nursing.
• Current valid North Carolina nursing license required.
• Basic Life Support (BLS) through American Red Cross or American Heart Association required.
• Driver’s License Class
