Registered Nurse Case Manager - Homecare What We Offer
A Home Care RN Case Manager coordinates comprehensive patient care in the home, assessing needs, developing individualized plans (POCs), and acting as a liaison between patients, families, doctors, and insurers to ensure quality, cost‑effective treatment, often supervising other home health staff and providing direct skilled nursing care.
What You’ll Do- Responsible for coordinating home care services which includes establishing and maintaining the patient’s plan of care.
- Customer service and financial management of your assigned caseload.
- Conducts admissions, recerts, ROC, and discharges for the assigned case load.
- Conducts routine nursing visits as needed to assess patient needs.
- Coordinate patient care between the patient, caregiver, physician, and other appropriate disciplines.
- Responsible for the completion of all home health documentation.
- Graduate of an accredited school of nursing required.
- Minimum Associate's or preferred Bachelor's degree in nursing.
- Current valid North Carolina nursing license required.
- Basic Life Support (BLS) through American Red Cross or American Heart Association required.
- Driver’s License Class
C. - Minimum 1 year of home health or related RN experience required.
