Primary Care Nurse Practitioner/Physician Assistant - Part-Time

Position Summary
Eventus Whole Health delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in‑house support staff.

Essential Duties and Responsibilities
Provide primary‑care services in accordance with Eventus Whole Health site‑specific protocols and consistent with the standard of care for the specialty of family medicine, geriatric medicine, and/or internal medicine as it pertains to long‑term care facility work.

Quality/Compliance

  • Review and sign MAR’s as requested by facilities; sign all Primary Care orders while at the facility and/or within 24 hours if faxed or emailed, including phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders as appropriate, and other relevant orders. Review and address labs within 24 hours of receipt. Manage all narcotic prescriptions specific to your facility’s patients in accordance with the Eventus Whole Health Narcotic Rx Policy.

    Provide the facility with adequate time each visit to allow for paperwork and administrative duties.
  • Provide on‑call triage duties as per the on‑call triage protocol. Triage policies and scheduling will be modified as needed; all providers must adhere to the newest agreed‑upon schedule. Eventus Whole Health triage policies will be based upon a majority vote of all involved providers.
  • Complete documentation and submit billing records per Eventus Whole Health charting and bill sheet submission policies. Provide the facility with a hard copy of the electronic documentation within 48 hours of service for inclusion in the patient’s facility medical record.
  • Establish and maintain open and positive communication with facility staff and administration. Provide verbal or written instruction or feedback regarding medications and other pertinent caregiver information. Meet with the Administrator or designated care coordinator prior to the start of your patient care visits each day and prior to leaving the facility each day.
  • Be knowledgeable of and adhere to Eventus Whole Health standards, policies, and procedures. Comply with all legal, regulatory, and professional ethical standards.
  • Maintain patient confidentiality at all times, including appropriate use of cell phones, emails, text messaging, patient charts, and EMR. Adhere to HIPAA rules and regulations.
  • Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding the care of a resident.

Qualifications / Education

  • Masters degree in Nursing from an accredited institution and a current, valid nursing license issued by the state in which you reside. All other required certifications must be current and valid.
  • Graduated from a physician assistant educational program accredited by the National Commission on Accreditation of Allied Education Programs.
  • Passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants.
  • Licensed as a Physician Assistant by the state in which you reside.
  • Specialization in geriatric, family, or psychiatry desired.

OR

  • Masters degree in Nursing from an accredited institution and a current, valid nursing license issued by the state in which you reside. All other required certifications must be current and valid.
  • Graduated from a physician assistant educational program accredited by the National Commission on Accreditation of Allied Education Programs.
  • Passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants.
  • Licensed as a Physician Assistant by the state in which you reside.
  • Specialization in geriatric, family, or psychiatry desired.
