Education and Outreach Specialist (Community-based)
We are excited to announce a new opportunity for a compassionate and dedicated Education and Outreach Specialist to join our multidisciplinary One Community team in the Southern Region of the Trillium service area.
As part of a collaborative, community-focused initiative, you’ll help connect individuals to vital resources while advancing our shared goal of helping people reach their full potential.
Your Role in Our MissionThe Education and Outreach Specialist serves as a bridge between community members and providers across the southern Trillium counties on the East coast of North Carolina.
Flexibility is essential; the position may require evenings, weekends, and holidays. This is a community-based position, and candidates must reside in Columbus County.
Responsibilities- Attend and facilitate community events, workshops, and outreach activities in Southern Trillium counties.
- Develop and deliver educational materials on health literacy, wellness, and available services.
- Build and maintain relationships with community partners, providers, and referral sources.
- Track outreach metrics and provide reports to the leadership team.
- Travel to counties:
Craven, Jones, Carteret, Sampson, Duplin, Pender, Onslow, New Hanover, Brunswick, Columbus, Bladen, Robeson, Hoke, and Sampson.
- High School diploma or GED required.
- Valid North Carolina driver’s license and insurance for personal and agency vehicles.
- Experience with community-based outreach or service planning preferred.
- Excellent communication, organization, and project management skills.
- Ability to work flexible hours including evenings and weekends.
- Hourly rate of $18.
