Position Summary

An employee in this position will screen incarcerated individuals for substance misuse and connect them with MAT/MOUD services as well as community resources identified for successful reentry. Therefore, this person will need to be able to work collaboratively, establishing relationships and maintaining connections with resources, in order to coordinate services with the Sheriff’s Office personnel, Justice Professionals, and Community Partners.

Duties And Responsibilities

Essential Functions of the Position

  • Performs and completes screening for substance misuse in order to identify goals that remove barriers for the participant to have successful reentry into the community.
  • Performs case management services.
  • Completes follow up of participants by remaining in contact with attorneys, court professionals, providers, and families for re-entry needs.
  • Maintains referral lists and serves as a referral specialist by monitoring and responding timely to referrals from the medical provider or other justice professional and following up on referrals submitted.
  • Identifies and facilitates access to services needed for each participant, such as developmental, behavioral, mental health, and substance misuse needs.
  • Functions as a substance misuse information and referral specialist.
  • Meets with participants prior to release to finalize their reentry plan that assists with connection to services that address identified needs as well as connects with community programs responsible for post-release services.
  • Coordinates re-entry services as identified to include but not limited to housing, transportation, insurance, etc.
  • Assists our medical provider within the Detention Center with continuity of care of MAT services as well as substance misuse and mental health needs.
  • Facilitates harm reduction strategies, such as Narcan training.
  • Performs accurate record keeping of all encounters, digitally with hard back-up.
  • Provides analysis data tracking for quarterly outcome reporting.
  • Prepares appropriate reports for public information related to substance misuse and opioid misuse for Harnett County.
  • Tracks inmate progress accurately through use of Excel Spreadsheets and/or data tracking software.
  • Understanding and experience of the insurance billing process.
  • Attends required meetings and/or trainings focused on MAT services, as needed.
  • Facilitates and develops partnership agreements that outline appropriate procedures for compliance with Protected Health Information (PHI) and Health Insurance Portability and Accountability Act (HIPAA) regulations.
  • Performs other related duties as assigned.
Minimum Qualifications

Requirements to Perform Work

  • Master’s degree from an accredited school of social work, counseling, mental health or related field.
  • Four (4) years of social work or counseling experience is preferred.
  • Possession of NC Professional licensure in relevant discipline of Clinical Social Work or Clinical Addiction Specialist or equivalent.
  • Valid driver’s license in the State of North Carolina.
