Licensed Practical Nurse (LPN) provides hands‑on resident care in an assisted living community, including assessments, vital signs, medication administration, treatments, documentation, and emergency response. The role involves assisting residents with activities of daily living, coordinating with physicians and pharmacists, mentoring team members, and participating in required training. Physical demands include frequent lifting and mobility assistance; candidates must hold current LPN/LVN licensure and certifications (CPR/AED/First Aid).

POSITION SUMMARY

Provides hands‑on resident care, proper and timely administration of medications and treatments, accurate verbal and written reports and rapid response to any/all emergency situations.

ESSENTIAL FUNCTIONS

Performs professional and restorative nursing services which include, but not limited to, resident assessments, recording of vital signs, administering medications and treatments.

Keeps timely documentation of written reports and verbal reports ensuring that quality care is provided.

Interacts with the resident's physician, pharmacist and medical personnel as needed.

Following the guidelines as outlined in the Resident Care Plan; assists residents with activities of daily living including bathing, dressing, eating, toileting, hygiene, and mobility.

Assists with in‑service and on‑the‑job training and mentoring of Team Members.

Participates in and attends all required in‑service training and education programs as scheduled.

Other duties as assigned.
