Position DetailsPosition Objective
To provide comprehensive case management and assessment services to the aging and disabled population of Chisago County. This position supports individuals through person-centered planning by assessing needs, determining eligibility for long-term services and supports, and developing service delivery plans that address environmental, social, safety, physical, rehabilitative, and health-related needs.
The Assessor / Case Manager conducts MnCHOICES assessments in accordance with state and federal requirements and collaborates with individuals, families, and community partners to promote independence, safety, health, and overall well-being. Work is performed under the limited supervision of the Social Services Supervisor. Caseloads and assessment duties are adjusted according to workload demands and organizational needs.
Essential Duties and ResponsibilitiesMnCHOICES Assessments
- Conduct person-centered MnCHOICES assessments for individuals seeking long-term services and supports, including home and community-based services.
- Determine program eligibility and service needs in compliance with Minnesota statutes, DHS policies, and federal regulations.
- Accurately complete documentation within required timelines using state-approved systems.
- Explain assessment outcomes, service options, and program requirements to individuals and their support networks.
- Maintain current knowledge of MnCHOICES tools, policies, and assessment procedures.
- Obtain and maintain eligibility as a certified MnCHOICES Assessor.
Case Management Services
- Provide ongoing case management services to assigned individuals in the aging and disabled population.
- Develop, implement, and monitor individualized service delivery plans that reflect person-centered goals and preferences.
- Coordinate and authorize services that address environmental, social, safety, physical, rehabilitative, and health needs.
- Monitor service effectiveness and revise plans as needs or circumstances change.
- Advocate for individuals to ensure access to appropriate services and supports.
- Work collaboratively with individuals, families, legal representatives, service providers, and community partners.
- Participate in interdisciplinary team meetings and care conferences as needed.
- Provide referrals and linkages to community resources, benefits, and support services.
- Promote self-determination, dignity, and cultural responsiveness in all interactions.
Compliance and Professional Standards
- Ensure compliance with county, state, and federal laws, rules, and regulations.
- Maintain accurate, timely, and confidential case records.
- Follow ethical standards and professional practices related to social services and case management.
- Participate in required trainings, meetings, and continuing education.
Minimum Qualifications
- Bachelor’s degree in Social Work, Human Services, or a related field (Master’s preferred or required per county policy).
- MnCHOICES Assessor certification or ability to obtain within required timeframe.
- Experience working with older adults and/or individuals with disabilities preferred.
- Valid Driver’s license and access to reliable transportation for travel to client sites.
Preferred Qualifications
- Experience conducting MnCHOICES assessments or other long-term care assessments.
- Knowledge of community-based resources, waiver programs, and county human service systems.

- Ability to interpret and apply complex state and federal program regulations.
- Excellent communication, interviewing, and interpersonal skills.

- Hybrid-work performed in office, community, and home settings.
- Regular travel within the county/region required.
