• Evaluates the patient and home health environment to determine required services
• Serves as the patient’s advocate by providing information
• Responsible for timely completion and synchronization of OASIS visits as required and developing the plan of care based on findings from evaluation visit
• Involves the patient and patient’s family in developing the plan of care

• RN state licensure in good standing
• Bachelor or Masters level degree in Psychology and experience in mental health
• Medicare, home healthcare, and OASIS experience
• 1 year of Psychiatric RN experience within the last 2 years
