Walmart seeks an Independent Optometrist to lease space in our stores and provide eye and vision care. You will assume a leading role in patient care, managing your own schedule and fees while maintaining high standards of service.

• Launch your services in Walmart stores.
• Take a leading role in patient care and general health and well‑being.
• Establish your days and hours of practice.
• Control your patient schedule and set your fee schedule.
• Keep 100 % of exam/professional fees.
• Optionally procure equipment/instrumentation and administrative/Opt-Tech support services if state allows.

• Doctor of Optometry (OD) from an accredited optometry school.
• Valid state optometry license.
• Ophthalmologist (MD or DO) from an accredited school.
• Valid state ophthalmology license.
• General Liability Certificate.
• Professional Liability Insurance Certificate.
• W‑9.
