Job SummaryAn Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or in an alternate infusion suite, ensuring safety and continuity under a physician’s plan of care.
Job Responsibilities- Initiate, develop, and implement intermediate nursing plan of care treatments, evaluating patient progress toward goals.
- Perform direct patient care, administer medications, and educate patients and families/caregivers.
- Modify treatment plans in response to changing patient status or physician orders.
- Assess patient needs and physical status at each skilled visit; reassess when needed.
- Obtain and clarify physician orders, informing physicians promptly of significant changes and providing written summaries.
- Train and mentor Infusion Nurse I’s as needed.
- Teach patients and families to achieve independence with prescribed therapy through active participation.
- Communicate effectively with the Clinical team, medical providers, patients, and caregivers.
- Prepare clinical documentation in real‑time; comply with billing timelines.
- Coordinate discharge planning and prepare discharge summaries with patient instructions.
- Demonstrate compliance with agency operations, policies, professional standards, and regulations.
- Maximize work efficiency by using computers and automation technologies.
- Respond promptly and appropriately to patient requests; initiate emergency procedures as necessary.
- Accept accountability for own practice through ethical and professional conduct.
- Observe legal and ethical guidelines for safeguarding patient confidentiality and HIPAA compliance.
- Explain the scope of services and financial obligations to patients.
- Attend required training, in‑services, and continuing education; maintain competency and licensure.
- Provide oversight and coordination of paraprofessionals in the home setting; may act as Supervisor when needed.
- Participate in multidisciplinary team conferences; mentor and precept other nurses.
- Participate in nursing department on‑call responsibilities.
- Perform other related duties as directed by supervisor.

BasicEducation and Experience
- Active and unrestricted RN license in the state of practice.
- Minimum of 1 year of nursing experience.
- Current CPR certification required.
- Demonstrated competency in patient care standards and infusion skill sets.
- Required licensure to operate a motor vehicle in the state of practice; access to a vehicle with proof of liability insurance.
- Basic computer literacy; ability to use nursing technology tools for communication and documentation.
- Lift up to 50 pounds (maximum 75 pounds); perform light lifting and various physical tasks such as walking, standing, crouching, kneeling, and reaching overhead.
- Exposure to blood, body fluids, and potential infectious materials; possible exposure to hazardous substances and unpredictable home environments.
- Safety requirements include closed‑toe shoes, facemask, goggles, gown, gloves, and a functioning communication device.
- Willing to travel up to 100% of the time for business purposes.
- Bachelor of Science in Nursing preferred.
- 1–2 years of previous infusion nursing experience preferred.
