RN - Transplant Coordinator, Renal – Sentara Health
Overview: Sentara Healthcare seeks a Full‑time Day Shift Transplant Coordinator for the Kidney/Pancreas Transplant team at Sentara Norfolk General Hospital. This 525‑bed Level 1 trauma center is the only transplant center in Hampton Roads and has performed more than 2,200 transplants. The hospital is Magnet® recognized for nursing excellence.

Location: Norfolk, VA
Work Shift: First (Days)
What you will do:
- Coordinate care with post‑transplant patients.
- Facilitate clinic flow, transplant evaluation, and patient care management.
- Provide weekly education sessions to patients and support members; arrange appointments with surgeons, social workers, financial coordinators, pharmacy, and dietary services.
- Admit patients, support them and their caregivers during the waiting period, and discharge transplant patients with clear home‑care instructions.
- Manage a caseload of patients and remain the primary point of contact throughout the transplant process, recovery, and home follow‑up.
- Hold a BSN with at least 2 years of acute‑care experience, strong renal knowledge, and an understanding of chronic disease. BLS required. Certified Clinical Transplant Coordinator (CCTC) required within 3 years of hire.
Outpatient clinic hours:
Monday – Friday, 8 am – 4:30 pm. The coordinator will have scheduled calls and an on‑call out of the office every 11th week.

Sentara Norfolk General Hospital is a 525‑bed tertiary care facility with the only Level I Adult Trauma Center and burn trauma unit in Hampton Roads. It is the primary teaching hospital for Eastern Virginia Medical School. The hospital provides comprehensive care in cardiology, neurosurgery, oncology, spine care, imaging, behavioral health, maternity, and neonatal services.
