The RCSU Case Manager provides short-term, intensive case management services to individuals admitted to the Residential Crisis Stabilization Unit. This role focuses on rapid assessment, service coordination, discharge planning, and linkage to community resources to support stabilization and successful transition to a lower level of care. The Case Manager works as part of a multidisciplinary crisis team in a fast-paced, high-acuity environment.
Essential Duties and Responsibilities Clinical & Case Management Functions- Conduct biopsychosocial assessments, needs assessments, and risk screenings in accordance with DBHDS and program requirements
- Collaborate with the clinical team to support individualized stabilization and discharge planning
- Coordinate referrals and linkages to community-based services, including outpatient treatment, housing, benefits, and social supports
- Provide brief supportive interventions and psychoeducation within scope of practice
- Advocate for individuals to ensure continuity of care and access to appropriate services
- Develop and document discharge plans beginning at admission
- Coordinate follow-up appointments and ensure warm handoffs to next providers
- Communicate effectively with families, guardians, CSBs, and external partners as appropriate
- Ensure discharge occurs within required time frames consistent with RCSU regulations
- Complete timely, accurate, and compliant documentation in the electronic health record (EHR)
- Maintain documentation consistent with DBHDS regulations, Medicaid requirements, and organizational policies
- Participate in utilization review and quality assurance activities as required
- Participate in multidisciplinary team meetings, shift handoffs, and care coordination discussions
- Support crisis response efforts as needed, including de-escalation and safety planning
- Follow all safety protocols and emergency procedures
- Bachelor’s degree in Human Services, Social Work, Psychology, Counseling, or a related field
- Eligible for or credentialed as a QMHP-A, QMHP-C, or QMHP-E in the Commonwealth of Virginia
- Experience working with individuals in behavioral health crisis, residential services, or acute care settings
- Ability to work effectively in a fast-paced, high-acuity environment
- Strong documentation, communication, and organizational skills
- Master’s degree in Social Work, Counseling, or related field
- Previous experience in crisis stabilization, inpatient, emergency, or residential behavioral health settings
- Familiarity with DBHDS crisis services regulations and Medicaid documentation standards
- Knowledge of crisis intervention principles and trauma-informed care
- Ability to assess psychosocial needs and coordinate appropriate services quickly
- Strong interpersonal and de-escalation skills
- Ability to work collaboratively within an interdisciplinary team
- Proficiency with electronic health records and basic computer applications
Work Environment Requirements
- Ability to remain alert and responsive in a crisis setting
- Ability to sit, stand, and move throughout the unit during shifts
- Ability to respond appropriately to individuals in emotional distress
- Successful completion of fingerprint-based criminal background check and Central Registry check in accordance with DBHDS and Virginia law
- Maintenance of required credentials and ongoing training
- Adherence to all organizational policies, confidentiality standards, and DBHDS regulations
