Urban Baby Beginnings® is seeking a dedicated Nurse Navigator to join our ReByrth™ team — a maternal-infant telehealth platform powered by UBB.
As a Nurse Navigator, you’ll provide a mix of virtual and in-person support to families before and after birth. Whether you’re meeting families at the hospital, offering postpartum check-ins via ReByrth™, or connecting them to community resources, you’ll be part of a movement that’s building safer childbirth cities — one family at a time.
Key Responsibilities- Care Navigation – Provide compassionate, individualized support to pregnant and postpartum clients, helping them understand and access available services
- Hospital-Based Outreach – Meet families at delivery hospitals, participate in clinical rounds, and engage directly at the point of care
- Virtual Platform Support – Guide families in using the ReByrth™ telehealth app for lactation, doula, mental health, and parenting resources
- Education & Coaching – Offer evidence-based education on prenatal care, postpartum recovery, infant development, and safety
- Follow-Up & Monitoring – Conduct ongoing contact with families through phone, video, and in-person visits to track needs and progress
- Collaboration – Work as part of a regional interdisciplinary team including doulas, lactation counselors, CHWs, peer support specialists, and hospitals
- Documentation – Complete timely documentation and data entry within our secure, EHR-integrated platform
- Outreach & Awareness – Support hospital and community outreach efforts to promote the ReByrth™ platform and UBB services
- Current RN license in Virginia (unencumbered)
- 2+ years of nursing experience
, preferably in maternal-child health, postpartum, pediatrics, or public health - Strong understanding of perinatal care, maternal mental health, and public health
- Excellent communication and empathy skills with a trauma-informed, culturally responsive lens
- Comfortable using telehealth and digital platforms
- Valid driver’s license and reliable transportation
- Willingness to travel across service areas
- Experience in hospital or community-based maternal health programs
- Familiarity with collaborative care models
This is not a remote position. Nurse Navigators must be available for video assessments & education, community engagement, and in-person family support.
