Job Overview

As a Social Worker at DaVita, you’ll be part of a team that values work‑life balance and prioritizes personal and professional growth. You will support and advocate for patients on dialysis, helping them understand their rights, manage the physical, mental, emotional, and financial demands of End‑Stage Renal Disease, and build meaningful relationships with patients and their families.

Responsibilities
  • Build meaningful and long‑term relationships with patients and their families in an intimate outpatient setting.
  • Support the team by advocating for patients and ensuring care plans meet CMS quality standards.
  • Provide emotional and practical support, including education on insurance and financial resources.
  • Assist patients in understanding rights and responsibilities during their dialysis journey.
  • Contribute to performance‑based rewards through individual and team achievements.
Qualifications
  • Meets all state‑required regulations to practice in the dialysis setting.
  • Master's degree in Social Work (MSW) with a specialization in clinical practice.
  • Licensed Master’s Social Worker or Licensed Clinical Social Worker.
  • Demonstrated knowledge of government and private insurance programs.
  • Basic computer skills in MS Word, Excel, PowerPoint, Outlook, and functional proficiency with DaVita applications within 60 days.
