Overview
Sentara is hiring an Inpatient Case Manager Social Worker, MSW – Days/PRN for the Resource Pool.
Required to work 4 hospitals in the Southside or Western Tidewater regions. Location and unit to be assigned based on greatest need. Must be within 75 miles and 90 minutes of all four hospitals in the assigned region.
Responsibilities- Work across assigned shifts in all units within the case management specialty; required to work in 4 hospitals in the Southside or Western Tidewater regions.

self-schedule at least 48 hours of prescheduled time per calendar month; of these, 16 hours should include availability to be scheduled on a weekend. - Develop and evaluate effective transition plans considering diagnosis, prognosis, treatment, services, and care continuum.
- Collaborate with patients, families, healthcare team members, and community organizations; advocate to identify needs and resources in hospital and community settings.
- Functions in Hospital-Based practice settings with potential regional or local travel and opportunities for growth.
Master's degree in Social Work required
Certification/Licensure- Certification in Case Management (CCM, CMAC or ACM-SW) required within one year of eligibility.
- BLS required within 90 days of hire.
- One year of related experience.
- Within 75 miles and 90 minutes of all 4 hospitals in the assigned region and willing to work shifts at any of the four based on greatest needs.
