Overview

The Kings Lynn and Coastal Community Therapy team has an exciting opportunity to recruit a full‑time Community Assistant Practitioner (37.5 hours per week).

The role is an autonomous practitioner position involving patient assessment, goal setting, implementation of treatment plans and primary prevention strategies. You will work under the direction of Occupational Therapists and Physiotherapists, but also have the flexibility to work independently.

The post covers the King’s Lynn and Coastal areas across West Norfolk, with your base at St James Clinic, King’s Lynn. A current UK driving licence and access to a vehicle is essential.

Responsibilities
  • Assess and treat patients with multiple conditions, sometimes complex in nature.
  • Prepare for, carry out and monitor assessments and treatments in specified clinical areas, and discharge in line with predetermined department protocols.
  • Modify and progress intervention / treatment using own clinical reasoning, notifying a qualified practitioner accordingly.
  • Monitor patients and promptly alert a qualified practitioner when there are unexpected changes.
  • Demonstrate problem solving, and contribute to the solution, working with colleagues.
  • Make clinical judgements on patients’ response to prescribed treatment, making adjustments as required, and report findings to a qualified practitioner.
  • Plan and prioritise own visits to patients and document on Systm One ledger.
  • Ensuring the outcome of patient visits are documented in Systm One record within 24 hours of visit.
  • Be flexible to accommodate admission avoidance patients.
Qualifications

You will need to demonstrate effective communication skills, problem solving skills and resilience to cope in changing and challenging situations. Previous experience working within a community setting or within a therapy‑based team would be desirable.

Supervision

Supervision will be provided by an Occupational Therapist or a Physiotherapist.
