Job Overview

We are looking for a highly motivated Community Health Care Assistant to join our community nursing team based in Fakenham and Holt. The role covers Fakenham, Wells, Holt and Sheringham GP surgeries, and will support patients in their own homes in rural Norfolk.

• Provide direct care to patients in their own homes within a team setting.
• Perform complex wound dressings, venepuncture, INR management, catheter insertion and management, insulin and dalteparin administration.
• Assist with health assessments, manage care plans and deliver treatments as part of a multidisciplinary team.
• Use IT systems effectively; training will be provided during induction.

• Experience working in a community setting is beneficial but not essential.
• Strong team‑working and communication skills.
• Ability to work autonomously and manage patient care independently.
