Provides direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization.
Responsibilities- Implements and monitors patient care plans. Monitors, records, and communicates patient condition as appropriate. Serves as a primary coordinator of all disciplines for well-coordinated patient care. Notes and carries out physician and nursing orders. Assesses and coordinates patient s discharge planning needs with members of the healthcare team. Conducts comprehensive assessments (initial, re-assessment, and focused) considering physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
- Establishes daily goals with the patient and family; formulates a plan of care; plans for transition or discharge.
- Implements nursing care according to the care plan; ensures continuity of care and documents care in the patient s record.
- Evaluates patient status and the effectiveness of nursing care; coordinates care with the healthcare team to manage the sequence and timing of patient care from admission to discharge.
- Delegation as part of care coordination.
- Minimum Requirements: Graduate of an accredited School of Nursing. Licensure and
Certifications:
Basic Life Support (BLS) - American Heart Association;
Must have a valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days. - Preferred Requirements: Bachelor of Science in Nursing;
Certifications include Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Program (NRP) or others as required by department.
Up to $10,000 Bonus Based on Eligibility
Anyone traveling more than 60 miles one way to Holy Cross is eligible for a travel stipend of $60 per day.
