This facility is seeking a Vascular Nurse Practitioner or Physician Assistant(NP) for locum tenens support as they look to fill a current need.

• Schedule:
  
  8 Hours Per Day between 8:00a-5:30p, 1-Hour Unpaid Lunch
• Practice Setting:
  Vascular Clinic
• Types of Cases:
  Pre-Op and Post-Op Vascular Surgery Patients In The Outpatient Vascular Clinic
• Credentialing Timeframe: 30-90 Days
• Electronic Medical Record (EMR): EPIC
• Certifications
  
  Required:
  
  Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS)
• Licensure
  
  Required:
  
  Active Indiana State License and Drug Enforcement Administration (DEA)
