An experienced Registered Nurse (RN) specializing in home health care is needed for a contract role in Concord, NC. This position requires a strong commitment to delivering compassionate and skilled nursing care in patients’ homes, with a focus on comprehensive assessments, personalized care planning, and patient education. Candidates must have OASIS experience to qualify.

• Provide thorough nursing care, including vital signs monitoring, medication management, and wound care in home settings.
• Conduct initial and follow-up patient assessments to evaluate health status.
• Develop and implement individualized care plans tailored to patient needs.
• Educate patients and their families on disease management, treatment adherence, and preventive health measures.
• Collaborate with physicians and other healthcare providers to coordinate patient care.
• Accurately document all care activities in the electronic medical records (EMR) system in a timely manner.
• Monitor and report any changes in patient conditions.
• Ensure all home health care practices comply with state and federal regulations.

• Active Registered Nurse (RN) license.
• At least 3 months of home health nursing experience utilizing OASIS.
• Experience with Homecare Homebase (HCHB) software is highly desirable.
• Clinical skills including IV therapy, wound care, and medication administration.
• Proficient in EMR technology.
• Strong communication and interpersonal abilities.
• Ability to work independently and manage schedules effectively.
• Valid driver’s license and dependable transportation.
