Key Responsibilities- Provide skilled nursing care and interventions in patients' homes
- Conduct thorough patient assessments and complete accurate OASIS documentation promptly
- Collaborate effectively with physicians and interdisciplinary care teams to implement tailored care plans
- Perform wound care, medication management, patient education, and monitor disease progression
- Report updates and changes in patient condition to clinical team members
- Active Registered Nurse (RN) license
- Minimum of one year recent experience in home health nursing
- Experience with OASIS documentation is required
- Proficiency with Home Care Home Base software is strongly preferred
- Ability to adhere to a full-time, structured schedule with daily patient visits
- Reliable transportation and a valid driver’s license
- Full-time, Monday through Friday
- On-call duties are probable
