We're hiring a dedicated Registered Nurse (RN) with OASIS experience for a 13-week full-time home health contract. If you're confident in your clinical skills, documentation accuracy, and ability to manage a set schedule — this is your opportunity to make a real impact.

• 13-week contract with the potential to extend
• Full-time schedule, Monday through Friday (on-call probable as well)
• Structured daily visit expectations

• Provide in-home nursing care and skilled interventions
• Conduct patient assessments and complete OASIS documentation accurately and on time
• Collaborate with physicians and care teams to deliver individualized care plans
• Perform wound care, medication management, patient education, and disease monitoring
• Communicate updates and changes in condition to the clinical team

• Active RN license
• Minimum 1 year of recent home health RN experience
• OASIS experience is required
• Proficiency with Home Care Home Base (strongly preferred)
• Able to manage a structured, full-time schedule with daily patient visits
• Reliable transportation and valid driver's license
