Experience:
Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty.
License:
Must have license in hand, no exceptions.
Certifications:
CPR, BLS
Must-Have:
Must have recent specialty experience.
Proficient in Home Care Home Base EMR and OASIS.
Must have reliable transportation, current driver’s license, and agency-required liability insurance.
Must be open to traveling up to 75 miles from assigned care center.
Must be comfortable accepting every patient on the schedule regardless of distance.
Must understand that there are no guaranteed hours with this contract.
Must complete all documentation within 24 hours post-appointment.
Description:
The RN will coordinate and direct the provision of home nursing care according to physician orders based on agency policies and procedures.
The RN will apply the nursing process competently.
The RN will be responsible to the Clinical Manager.
The RN will directly supervise and evaluate Home Health Aides and Licensed Practical Nurses or Licensed Vocational Nurses.
Home visits are required as part of the role.
Periodic office meetings will be held.
The RN will be assigned between 5 to 10 patients daily, equating to approximately 6 to 8 productivity points.
Patients may include start of care, wound care, and follow-up visits.
The RN must be available for on-call rotations, typically one weekend and two weekdays per month.
All travelers are expected to follow productivity expectations to maintain financial needs.
The RN will be compensated for travel between patient homes, but the commute to and from the first and last patient is not billable.
