We are seeking a highly experienced and motivated Practice Nurse to join our supportive multidisciplinary team. This is an excellent opportunity for a confident clinician who enjoys working autonomously while delivering high-quality, evidence-based care.
The role is suited to a nurse with significant hands-on experience in primary care, general practice or community nursing, who is comfortable managing patients independently while contributing to a collaborative clinical team.

Main duties of the jobMain Duties
Chronic disease management (e.g. diabetes, asthma, COPD)
Assessment and treatment of minor illness, minor ailments, and injuries
Clinical assessment, diagnosis, treatment, and evaluation of care
Preventative healthcare and health promotion
Safe clinical decision-making and contribution to policy implementation
Collaborative working within the general practice team

Being near the Marches border we have the advantage of an attractive rural environment with good links to the national motorway network providing access to both rural hills and coast in one direction and access to the larger urban areas of Shrewsbury, Chester and Birmingham within a reasonable drive time in the other direction.

. We are based in modern, purpose-built premises and use EMIS Web and Docman 10 clinical systems.
The Practice Nurse will deliver high-quality, evidence-based nursing care to the practice population, working both autonomously and as part of a multidisciplinary team. The role includes supporting new-to-practice nurses and other healthcare professionals, helping to strengthen integration across primary care, community, secondary, and social care services.
The post-holder will provide care across key clinical areas including chronic disease management, immunisations, cervical screening, infection prevention and control, health promotion, and public health screening programmes. They will also contribute to population health management, supporting initiatives that improve patient access, reduce health inequalities, and enhance health outcomes.
As an integral member of the team, the Practice Nurse will promote clinical best practice, participate in training and professional development, and support the review and implementation of clinical policies and procedures. Appropriate clinical supervision and day-to-day support will be provided by the organisation.
Qualifications- Registered Adult Nurse with the Nursing and Midwifery Council (NMC)
- Minimum 5 years clinical experience in primary or community care
- Recognised Practice Nurse qualification (degree level)
- Experience in chronic disease management and minor illness
- Ability to work autonomously within a multidisciplinary team
- Post-registration professional development in primary care, long-term condition management (e.g., asthma, diabetes), immunisation, cervical screening, etc.
- Chronic disease management courses or certificates.
- Independent nurse prescriber qualification
- Additional specialist practice nurse experience or training
- Experience of working within a nursing team
- Significant hands on nursing experience
- Experience of working in primary care or community
