Overview

Hospice RN Case Manager role y’s Home Care & Hospice, part of Trinity Health, Bucks County, PA. We provide compassionate, at-home care and are growing with a new strategy, vision, and technology. We offer the Home Care Connect virtual care program to help patients avoid preventable ER visits and hospitalizations.

St. Mary’s Home Care & Hospice is seeking a full-time Hospice RN Case Manager.

Responsibilities
  • Provide professional nursing care to hospice clients as prescribed by the physician and in compliance with the Nurse Practice Act, licensure requirements, and organizational policies.
  • Fully responsible for client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes.
  • Serve as the case manager for hospice patients and collaborate with the physician and interdisciplinary team.
  • Anticipate and manage patient symptoms; identify family/caregiver grief/loss issues; assist in implementing the Hospice plan of care.
  • Provide functional support to Hospice Aides and LPN/LVNs as needed.
  • Case-manage a team of hospice patients; participate in weekly Interdisciplinary meetings.
  • Interact with the primary care physician and/or hospice medical director as needed to manage patient symptoms.
Qualifications
  • Graduate of an approved nursing education program
  • Licensure as a Registered Nurse in the State of Pennsylvania
  • 2 years experience as a professional nurse in acute care, home care or ER
  • Home care experience preferred but not required
  • EPIC experience preferred
