St. Mary’s Home Care is seeking a Licensed Social Worker for a new Hospice agency in Bucks County, PA.
Employment TypeFull time
Sign‑On Bonus$5,000 sign‑on bonus offered. Excluded from bonus are current employees or previous employees who have left within one year or outside sourcing agencies.
Education & LicensureMaster's Degree from a school of social work accredited by the Council on Social Work Education. Currently licensed to practice as a Medical Social Worker in Pennsylvania.
ExperienceTwo (2) years of social work experience in a health care setting.
Responsibilities- The Medical Social Worker is responsible for the provision of consultation and services to homebound individuals in the need of assistance with the solution of complex social and emotional problems, which impede management of medical regimen.
- Master's degree graduate of an approved clinical education program
- Current clinical license in the state of Pennsylvania
- Must have a minimum of two (2) years' experience of professional clinical experience in a home care setting
- Must have current Driver’s license and reliable transportation to and from work site
- Ability to consistently demonstrate a commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program
