Family Promise of Sussex County has been addressing homelessness in the community for 25 years, growing from a grassroots effort into a comprehensive service provider. The organization is dedicated to helping individuals and families secure sustainable housing, guided by the belief that everyone deserves a home. Their services range from homelessness prevention and emergency shelter to intensive case management and permanent supportive housing programs.

With a mission to ensure housing stability, their impactful programs also include employment counseling, utility assistance, and rapid rehousing for those in need.

Family Promise of Sussex County is looking to hire a full-time case manager to provide information, resources, and referrals through one-on-one case management and group sessions with clients. Case managers work as a part of the team in moving clients from initial contact through to housing permanency.

Responsibilities include, but are not limited to:

  • Assisting clients in completing agency intake, assessments, and program applications, providing education and guidance on agency and programs
  • Providing financial and employment counseling, life skill seminars, and other relevant training sessions. Working with clients to develop individual goals focused on meeting household needs
  • Assisting client households in applying for disability, utility assistance, and other local benefits
  • Establishing rapport with clients, and obtaining and documenting the essential data as required by program funders andthe agency
  • Interpreting data to establish and substantiate eligibility for services
  • Accurately assessing risk to self, others, or property, and the presence of a grave disability
  • Utilizing appropriate crisis intervention techniques for client stabilization
  • Providing appropriate referrals for client stabilization
  • Developing and documenting appropriate safety plans
  • Completing all required progress notes according to agency comprehensive guidelines, policies, and procedures regarding format, content, and timeliness
  • Collaborating with clients to build a strong, structured, and professional relationship while utilizing a variety of techniques and processes
  • Exhibiting knowledge of various referral resources within the community to meet client needs

All agency staff are responsible for engaging in special events and fundraising activities

  • Minimum Bachelor's Level degree in Social Work or related field, MSW preferred
  • Minimum three years of experience in case management and working with impoverished, at-risk populations
