Compensation & BenefitsCompensation: $97,000.00 per year
Medical, medical stipend, dental, vision, benefit time, 401K, tuition reimbursement.
ScheduleSchedule M-F 8:30 a.m.
- 4:30 p.m. Admin on call rotation.
- Consults with physicians, hospice team members, patients and/or family members while establishing a coordinated hospice referral.
- Communicates effectively with patients, family, staff and physicians in order to ensure delivery of quality care and services.
- Ensures professional staff’s compliance with mandatory educational programs and health‑related requirements.
- Works with the QAPI and Education staff to provide the education component of nursing meetings.
- Ensures all chart audits are complete in a timely manner per regulations. Performs random review of 10% of all open charts monthly.
- Reviews all documentation in patient charts to ensure it meets the standards of the Conditions of Participation, CHAP and NJ DHSS.
- Makes field supervisory visits with staff, including per diem and on‑call staff, annually.
- Ensures compliance with Medicare, NJ Department of Health and Senior Service regulations and CHAP regulations.
- Facilitates cohesiveness and open communication within the organization.
- Serves as a customer service advocate, facilitating rapid response to all referrals and patient needs.
- Communicates with Intake Coordinator to prioritize admissions on a daily basis; coordinates patient referrals with RN Case Managers.
- Review and ensures accuracy of new admission chart generation and computer data; reviews and gives 2nd level approval to all hospice 485’s.
- Responsible for coordination of care and case management activities, including on‑call staff; provides administrative backup for on‑call; maximizes efficiency of patient care through monitoring of staff assignments.
- Follow up with DME companies to confirm removal of equipment; reviews and prepares MD discharge summary for distribution to MDs.
- Collects and maintains statistical data on all referred patients and submits same monthly to Administration.
- Ensures face‑to‑face encounters are completed within the required timeframe.
- Participates in all required in‑service education programs and meetings.
- Demonstrates a commitment to own work schedule by adhering to attendance standards. Provides proper notification in advance for absence and tardiness.
- Adheres to employment policies in accordance with the Employee Handbook.
- Demonstrates compliance with the dress code policy by appearing well groomed, wearing while on duty and maintaining a professional appearance.
- Performs other duties, as assigned.
- Leadership ability: including evidence of ability to take initiative, delegate authority appropriately, ability to role model, ability to serve as a change agent.
- Ability to perform in a high‑stress position with full responsibility for hospice operation.
- Solid analytical decision‑making and problem‑solving skills.
- Ability to handle detailed, complex concepts and problems and to make rapid decisions regarding administrative issues.
- Ability to effectively lead others and to establish strong and appropriate supervisory relationships.
- Ability to develop constructive relationships with colleagues, and outside agencies, organizations, and individuals.
- Ability to maintain a flexible work schedule to meet various demands of executive management in hospice.

- Conveys a professional and positive image and attitude regarding the hospice organization.
Bachelor of Science in Nursing preferred from an NLN accredited program; licensed to practice as a Registered Nurse in NJ, and PA.
Preferred EducationCurrent New Jersey and Pennsylvania Nursing License.
Required ExperienceThree (3) years of experience in a Medicare‑certified Hospice/Home Care agency under qualified nursing supervision.
Preferred ExperiencePrior clinical supervisory experience.
