We are seeking a compassionate and dedicated Case Manager to join our team. Asteroid Health is a modern outpatient mental health program offering OP, IOP, and PHP services. The Case Manager provides comprehensive, client-centered case management to adults with mental health needs, ensuring they are connected to the right care, at the right time, with the right supports.
Duties- Conduct comprehensive assessments to identify client needs related to behavioral health, addiction, child welfare, and other social services.
- Develop individualized care plans tailored to each client’s circumstances and goals.
- Provide crisis intervention and crisis management support to clients experiencing urgent situations.
- Utilize motivational interviewing techniques to encourage positive behavioral changes.
- Coordinate with community resources such as Section 8 housing, Medicare services, workforce development programs, and childcare providers.
- Monitor client progress and adjust care plans as needed to ensure effective outcomes.
- Offer mentoring and supportive guidance to clients, fostering independence and resilience.
- Supervise or collaborate with other staff or volunteers involved in client care when applicable.
- Maintain accurate documentation of all interactions, assessments, and service plans in compliance with organizational policies.
- Proven experience in social work, behavioral health, or related fields such as child welfare or workforce development.
- Strong skills in crisis intervention, crisis management, addiction counseling, and motivational interviewing.
- Supervising experience is preferred but not required; leadership qualities are a plus.
- Knowledge of Section 8 housing programs, Medicare services, and child welfare systems highly desirable.
- Excellent communication and interpersonal skills with the ability to build rapport with diverse populations.
- Compassionate approach with a commitment to client-centered care.
- Ability to handle sensitive situations discreetly and professionally.
- Relevant certifications or licenses in social work or counseling are advantageous but not mandatory.
