Registered Nurse - Home Health

We are seeking a compassionate and skilled Registered Nurse to provide home health care to clients within designated geographic territories. The ideal candidate will follow personalized care plans, administer treatments, and coordinate care management, ensuring quality patient care with empathy and strong communication skills.

Key Responsibilities

• Follow care plans tailored to individual patient needs.
• Conduct home visits within assigned geographic areas.
• Administer medications, wound care, and other necessary treatments.
• Monitor patient conditions and report changes to the Clinical or Client Services Manager.
• Accurately execute and document doctors' orders.
• Conduct client assessments as needed.
• Manage and coordinate care delivery effectively.
• Utilize a touchpad tablet for documenting observations, interventions, and evaluations.
• Provide compassionate and empathetic care to clients and their families.
• Take and track vital signs, measure, and administer prescribed medications.
• Offer wound and catheter care as required.
• Assist with feeding, bathing, and maintaining patient hygiene.
• Maintain clean bedding and manage supply tracking.
• Work varied shifts as needed.
• Ensure reliable transportation to reach clients' residences.
• Deliver nursing care independently and confidently.
• Establish rapport with patients and their families through clear and compassionate communication.

• Associate's degree in Nursing (required).
• Minimum of 3 years of nursing experience in a residential setting.

• Registered Nurse (RN) License (required).
• Basic Life Support (BLS) Certification (preferred).
