*** We are seeking a compassionate and dedicated Registered Nurse (RN) to join our hospice care team as a Case Manager. In this role, you will coordinate and deliver skilled nursing care and end-of-life support to patients and their families in home and facility settings.

* Key Responsibilities:

*** Assess patient needs and develop individualized care plans.
* Educate patients and families on hospice philosophy, comfort measures, and medication management.
* Monitor and document patient conditions, adjusting care plans as needed.
* Participate in on-call rotation and provide after-hours coverage as needed.## Qualifications
*
* Required:

** Current unrestricted State of Kansas License to practice as a Registered Nurse. Knowledge of professional nursing theory, standards of nursing practice, scope of practice and relevant statutes and regulations. Ability to prioritize work, meet deadlines, handle multiple projects, pay strong attention to detail and accuracy and work under pressure in a cooperative work environment. Must be self-directed, motivated and flexible.

Must possess a valid Kansas Driver’s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standard as defined by Community Care Hospice policy. Availability of reliable transportation is required for this position.
** Preferred:
** Home Health or Hospice experience preferred. One year professional medical-surgical experience preferred.
