Skilled Nursing LPN needed in Newport, Vermont to provide safe, hands on resident care in a skilled nursing setting. This role includes medication administration, treatments, monitoring and reporting changes in condition, supporting care plans, and accurate documentation while working closely with the nursing team to maintain quality care and a smooth shift.
Active Vermont LPN license required (or eligible to obtain). Skilled nursing, LTC, or rehab experience preferred. Strong communication, reliability, and a patient focused approach are important. Weekend and holiday rotation may apply depending on facility needs.
