RN - Hospice

Monday - Friday, 8 am - 5 pm with some weekend coverage required. Some call may be scheduled, but only on an emergency basis.

We are seeking a dedicated Home Health and Hospice Nurse to provide exceptional patient care in their homes. The ideal candidate will be experienced in OASIS assessments and will have a strong background in home health and hospice services.

• Conduct home visits and provide comprehensive nursing assessments.
• Manage patient care plans and coordinate services with other healthcare professionals.
• Document patient progress and update electronic health records using Health Wyse.
• Adapt to changing schedules and prioritize patient needs.

• Must have OASIS (Outcome and Assessment Information Set) experience.
• Home health and hospice experience required.
• Experience traveling from patient to patient for home visits.
• Must have a driver's license in good standing.
• BLS certification required.
