** Join Our Team as a Home Health RN!** - Embrace the opportunity to make a meaningful impact in patients' lives as a Home Health Nurse.- Enjoy a fulfilling Dayshift schedule:
- Monday - Friday, 8am - 5pm - Some weekend coverage required - Emergency call may be scheduled on rare occasions

* Key Responsibilities:
** - Conduct home visits, providing compassionate care to patients in their homes.
- Utilize your OASIS experience to complete assessments and documentation effectively.
- Manage and adapt to the dynamic nature of home health schedules.
- Collaborate with a dedicated team to deliver top-quality patient care.
** Requirements:** - Must possess OASIS (Outcome and Assessment Information Set) experience.
- Home Health and Hospice experience is essential.
- Proven experience in traveling from patient to patient for home visits.
- Valid driver’s license in good standing is required.
- Basic Life Support (BLS) certification is necessary.
** Additional Information:** - Familiarity with EMR systems, specifically Health Wyse (Careficient), is preferred.
- Engage in outpatient care with a supportive and dynamic team.
- Background checks will be conducted by the facility.
