Summary

Reports to the Pharmacist-in-Charge. Under general supervision of a Pharmacy Supervisor is responsible for the interpretation, evaluation, preparation and dispensing of medications pursuant to a legitimate order in keeping with established policies, procedures, and standards of practice and scope of pharmacy services of the respective affiliate. Collaborates with multidisciplinary care providers to monitor, assess, and intervene as needed to ensure safe, appropriate, and cost-effective medication use.

Oversees and closely reviews the work of pharmacy technicians and ancillary support personnel. Responsibilities
  • Know and act in accordance with the principles of the Brown University Health Corporate Compliance Program and Code of Conduct.
  • Receive patient medication orders as written by a physician or practitioner. Review and interpret each order to determine completeness, therapeutic appropriateness relative to the patient s clinical status, concomitant drug therapy and diet, and with consideration of aging processes and developmental stages. Contact the prescriber or covering prescriber as necessary to initiate interventions to ensure therapeutic efficacy, minimize risks, and optimize cost effectiveness.
  • Apply pharmacotherapeutic and pharmacokinetic principles to ensure safe and effective use of medications for the patient populations served.
  • Provide drug information and consultation to prescribers, nurses, and other health care professionals regarding drug therapy, including interactions, side effects, incompatibilities, allergies, order discrepancies, stability issues, and administration.
  • Proactively review and approve medication orders in Life Chart including drug, dose, dose form, frequency, and duration of therapy. Ensure relevant patient-specific parameters are present (e.g., diagnosis, known allergies, weight, age).
  • Supervise work performed by pharmacy interns and technicians to ensure performance standards and compliance with federal, state, and organizational rules and regulations.
  • Identify, document, resolve, monitor, and prevent drug therapy problems, including contraindications, duplications of therapy, and interactions.
  • Engage with other clinical pharmacists or clinical pharmacist specialists for complex clinical situations.
  • Integrate patient-focused medication therapy management and pharmaceutical care in accordance with established clinical pathways or guidelines.
  • Participate in prospective and retrospective pharmacokinetic dosing and monitoring of selected medications and recommend dose adjustments and laboratory monitoring as required.
  • Ensure compliance with target drug programs, formulary management initiatives, medication management initiatives, and adverse drug event monitoring programs.
  • Document and report all clinical interventions, medication errors, near misses, and adverse drug events per established protocols.
  • Participate in emergency code blue response and in daily medical, multidisciplinary, and specialty rounds as time allows.
  • Review and approve formulations for sterile products requiring compounding or manipulation prior to dispensing. Complete in-process and final checks for medications prepared by pharmacy technicians to ensure accuracy, sterility, and stability of compounded products.
