Overview

Position:
Community Health Nurse.

Location:

Depoe Bay, Oregon (Lincoln County). This role involves direct patient care and treatment in primary care clinics, VA, or School-based Health Centers (SBHC). The person in this position promotes health through education, care coordination, and health maintenance.

Compensation: $40.19 - $54.02 per hour (as listed). Lincoln County follows Oregon Pay Equity laws in reviewing education and experience for wage offers. Lincoln County offers a comprehensive benefits package including medical, dental, and vision coverage; 11% County contribution to 401(k);
County funded Health Savings Account (HSA); $40,000 County paid life insurance;
Employee Assistance Programs (EAP).

Essential Functions / Major Responsibilities

Community Health Nurses will follow the six standards of practice:

  • Assessment – Collect comprehensive data related to the health of the population served. Document assessment data in terms understandable to all involved in the care process.
  • Diagnosis and Priorities – Analyze assessment data to derive a nursing diagnosis and priorities. Document diagnoses or concerns to facilitate the population’s involvement in developing a plan of care.
  • Outcome identification – Identify outcomes that are culturally relevant, health literate, evidence-based, and attainable through local resources and policies. Document expected outcomes and measurable objectives.
  • Planning – Develop a client-centered plan reflecting best practices by identifying strategies, action plans, and alternatives. Document the plan using health-literate and culturally competent language.
  • Implementation – Oversee the plan’s implementation through treatment, case management, referrals to community partners, collaboration with community resources, health promotion, and education. Document activities and status of care coordination.
  • Evaluation – Apply ongoing assessment data to revise plans, interventions, and activities to achieve health outcomes. Document changes to enhance effectiveness of interventions.

For Primary Care and School-based Health Center CHN, additional responsibilities include:

  • Apply theories and principles of primary care and adolescent health nursing practices in clinics and schools.
  • Set appropriate priorities to provide quality patient care; utilize staff feedback; maintain confidentiality of patient information.
  • Perform direct care activities by administering medications, observing and documenting patient responses and side effects; administer interventions according to standing orders and protocols.
  • Provide a caring, respectful environment; contribute to a high-performance clinic environment with positive leadership and teamwork.
  • Coordinate with other clinic and departmental personnel to resolve operational problems and provide high-quality patient care.
  • Meet training requirements according to HRSA, HHS, and OHA guidelines.
  • Participate in ongoing quality improvement and use data reports to guide practice.
  • Establish professional rapport with clients, community, and health care partners to promote health.

Other duties as assigned:

  • Attend and participate in team meetings, trainings, and activities to further community health.
Typical Qualifications

Minimum Education Level
Associate degree in Nursing from an accredited school of nursing.

Minimum Experience Level
Three years of nursing experience. Practicums in Nursing programs and lived experience may substitute for years of experience.

Driver’s License
Oregon Driver's License required within 30 days of hire.

Other Licenses
Must possess a valid license to practice in the State of Oregon as a Registered Professional Nurse as defined by statutes or administrative rules, which include but are not limited to ORS 678.010 to 678.360 at the time of appointment. Maintain current and active licensures and certifications, as applicable, and provide verifications at renewals.

Desirable Qualifications
Bachelor of Science degree in Nursing (BSN) with one year of nursing experience, preferably in a primary care position.
